With over 6,000 individuals experiencing homelessness in the city, of which 75% are unsheltered, it is essential to provide support and assistance to those in need. The San Jose Citywide Homeless Outreach & Engagement program is dedicated to helping homeless families and individuals find the support they need to rebuild their lives. Our outreach teams proactively visit various encampments and engage with homeless residents to offer assistance. We prioritize helping individuals find emergency shelter, showers, laundry services, and meal programs. Moreover, we provide access to medical services to ensure their well-being is taken care of. A key component of our efforts is the provision of case management services. Through this, we work closely with individuals and families to develop personalized plans that address their unique circumstances and help them secure stable housing. We believe that by providing the necessary resources and support, we can help individuals transition out of homelessness and into permanent homes.
